OHSIRL Country Choice Request Form

Please NOTE:
A. Each school may ask for as many countries as they will need. We encourage schools to as much as possible fully staff a country before requesting another.

B. The guideline is only one delegate from a country in any given committee -- that means that depending on the country a delegation may have to fill 19 slots. The exception to this rule is the Big Five, which should have 2 delegates per committee. This is a general guideline only.

C. Please list more countries than you desire in case your first choices are already taken. Remember, you may not represent a country your school represented last year. Fill out the form and click the "Send this form to MUN" button and it will be routed to Jim Savard. He will notify you of your country choices. If for some reason you request a country and cannot fill it, please let Jim Savard know.

D. Do NOT hit the ENTER key until the form is COMPLETELY filled in! USE THE TAB KEY TO MOVE THE CURSOR TO THE NEXT LINE! Using the ENTER key before the form is COMPLETELY filled in will send the form to the MUN before you've finished!

E. Only when the form is finished, press the ENTER key or click the "Send this form to MUN " button at the bottom of the form.
